I acknowledge that the MBA qualification from the University of Kelaniya is the primary qualification relevant to my eligibility for membership, and I understand that the Association may verify my MBA status against appropriate University or Association records.

I accept full personal responsibility for the accuracy, authenticity and consequences of all other information and qualifications declared by me. I understand that the Association is entitled to rely on my declarations and is not responsible for independently verifying information that I provide, unless it specifically decides that verification is necessary.

I further acknowledge that if any information provided by me is subsequently found to be false, misleading, fraudulent, materially incomplete, or otherwise capable of causing reputational, financial, legal or other liability to the Association, the University of Kelaniya, its officers, members or any third party, I shall bear responsibility for my statements and the consequences arising from them.

I agree to cooperate with any reasonable request by the Association for clarification or supporting evidence concerning information that I have declared. I understand that the Association may take appropriate action under its Constitution, rules, policies and applicable law if material misrepresentation or misconduct is established.
